Checklist for opening a restaurant, change of ownership of existing restaurant or remodel of existing restaurant
Complete and submit all the following forms to FCEHS
- Food Service Application for restaurant or base of operation
- Mobile Foodservice Application Operation in other Counties
- Food Service Application Addendum Plans drawn to scale**
- Equipment Specification **
- Water Interruption Emergency Plan
- Menu
- Verification of Residency Form
- Key Drop Agreement
- Requirements for dogs on patio
- Checklist for Food Service Application
- Base of Operation checklist
- Base of Operation not in Forsyth County checklist
- Opening inspection checklist - operator
- Mobile Food Service application checklist
- Mobile Food Service Location Listing Form
- Mobile Food Service Toilet Use Agreement
- Mobile Food Service Propoerty Use Agreement
**May not be required if previous plans exist in office file and no changes are made to existing equipment and restaurant layout
Once all of the above documentation is submitted, allow 10 business days for a response from EHS
