Food Service Program
The Environmental Health Department is responsible for the inspection of food service facilities in Forsyth County. Email questions to Alan Zee
If you are experiencing a water interruption at your facility, please contact Alan Zee immediately along with Forsyth County/Cumming Water Department.
Complete and submit all the following forms to FCEHS
Food Service Application Packet
- Cover Letter
- Food Service Application Checklist
- Food Service Application for restaurant or base of operation
- Food Service Application Addendum
- Verification of Residency Form
- Floor Plan: Plans drawn to scale**
- Manufacturer Specifications**
- Menu
- Food service plan review and permit fee (based on number of seats)
**May not be required if previous plans exist in office file and no changes are made to existing equipment and restaurant layout
All documents listed above must be included with the application.
The documents listed below should be submitted during plan review or prior to opening inspection
- Copy of business plan (summary of how you plan to operate)
- Emergency Operation Plan: If the facility will continue to operate in the event of a water interruption (see application)

Once all of the above documentation is submitted, allow 10 business days for a response from EHS
